Longstanding shoulder problems often produce a scattered collection of records: an ultrasound, several prescriptions and an MRI after another difficult period. Before an orthopaedic consultation, connect those records into a timeline and explain what your arm currently prevents you from doing.
Identify the affected shoulder and your dominant hand. Record the approximate onset, any injury and changes in work or training. AAOS information on shoulder problems describes several possible causes of pain that require assessment. A matching description online does not establish your diagnosis.
Mark the important turning points: improvement, another injury, a new limitation or an intervention. If you cannot recall an exact date, a month or the order of events is sufficient. The aim is a readable clinical history, not a reconstruction of every day over several years.
Choose examples that need no specialist vocabulary: putting on a coat, reaching a shelf, washing your hair or carrying a bag. Explain whether pain, weakness, restricted movement or a feeling of insecurity is the main obstacle. Mention sleep and desk work when relevant to your concern. Do not force a painful movement for demonstration; the clinician can decide how to examine you appropriately.
Include records of previous dislocations, operations or problems in the other shoulder. State your current goal separately, whether that is working, dressing independently or understanding recurring difficulties during sport.
Collect existing X-rays, ultrasound and MRI examinations together with specialist reports. Check the side, date and readability of each item. Share original images where available, alongside their descriptions. Before an orthopaedic consultation in Turkey, confirm the accepted format and transfer method. Do not arrange new imaging simply because another clinic uses a different scanner; ask what additional question it would answer.
For each treatment, record the period used, what changed and why it ended. If injections were given, try to obtain the medicine, site and date. For rehabilitation, describe the appointment format and whether you could follow the programme. “Physiotherapy did not help” leaves important details unclear without this context. Share your medicines, allergies and other health conditions without changing prescribed treatment yourself.
Ask which explanations the specialist considers likely, what supports them and what remains to be established. Request the proposed sequence of actions and review criteria in writing. If you are considering travel for treatment, clarify whether the main question can be addressed in one visit or needs further stages. An organised history makes more consultation time available for decisions while preserving the role of examination and individual assessment.
