Planning rehabilitation after neurosurgery

Rehabilitation needs after neurosurgery vary. Before the operation, agree how those needs will be assessed, who will explain the goals and how the programme will continue after discharge. A universal collection of exercises cannot answer those questions.

Record the starting level of independence

Describe what the person manages independently and where help is already needed. Include mobility, dressing, communication, reading instructions and household responsibilities. This is an account of daily life for the team, not an invitation to perform online neurological tests. Ask who will conduct the professional assessment and what information is useful beforehand.

List any assistive devices currently used. Explain who helps at home and when that support is available. The actual circumstances need to be understood before discussing the next rehabilitation setting.

Match professionals to the assessed tasks

NHS information on physiotherapy describes work on movement and function, including after surgery. The appropriate combination of support is individual. Ask whether mobility, self-care, communication or other difficulties need assessment and which professional covers each area. One available service does not establish that every need has been addressed.

When planning neurosurgical treatment in Turkey, confirm the relevant assessment at the selected hospital and how it continues beyond admission. A service listed generally still needs confirmation for your proposed programme.

Coordinate restrictions and teaching

Request consistent written instructions from the surgeon and rehabilitation professionals. They should explain permitted activities, restrictions and how these will be reviewed. If a long document is difficult to use, ask for a short accessible version and demonstrations. A companion can attend teaching with the patient’s agreement and should have an opportunity to ask practical questions.

Do not choose exercises from another person’s operation story. Establish how to contact the team when an instructed task is difficult or an explanation remains unclear after discharge.

Prepare the move home or to another centre

Ask who determines the appropriate next setting and the conditions that must be in place. Describe accommodation, travel and available assistance. Request records of the procedure, current medicines, restrictions and functional assessment. Arrange the first contact with the local team and the document handover. Within your medical travel plan, allow for a changed route if the assessed needs differ from expectations.

Establish how the programme is updated

Identify responsibility for review and ask how to record practical changes between appointments. Clinicians determine the next assessment and permitted activity. Obtain separate instructions for urgent concerns and out-of-hours contact. If care is shared between countries, ask which team holds the current programme and circulate dated updates. This helps prevent an older instruction from continuing after the plan has changed.
