CT colonography after an unsuitable colonoscopy: questions before booking

CT colonography can be an option when conventional colonoscopy is unsuitable or could not be completed. Its usefulness depends on the question the clinician needs answered. It creates images of the large bowel but cannot take a biopsy or remove a polyp during the scan. Before booking, understand both the examination and what would follow a significant finding.

What “virtual colonoscopy” means

The word virtual describes how the images are produced. It does not mean that there is no preparation or that the examination has no medical limitations. A small tube is placed in the rectum, gas expands the bowel and CT images are obtained. The examination uses X-rays. The radiology department determines the preparation and whether contrast agents are part of the particular protocol.

CT colonography is different from an ordinary abdominal CT. A referral that says only “CT” does not establish that a dedicated large-bowel examination will be performed. Compare the exact name on the referral with the service being offered before paying. This detail is particularly useful when documents are translated between Russian, English and Turkish.

Whether the alternative answers the original question

The reason conventional colonoscopy is unsuitable should be explained in the referral. The team needs to decide whether imaging can answer the clinical question and what further investigation would be required if it reveals an abnormality. Endoscopy may need to be considered again if tissue sampling is necessary, because the scan cannot provide that specimen.

CT colonography is not a universal workaround for every bowel problem. Tell the team about active inflammation, severe pain, previous bowel injury and any possibility of pregnancy before booking. These circumstances can affect suitability. New severe pain or marked cramps on the day requires local medical assessment first; do not proceed with a planned CT colonography while in that condition. Choosing the examination simply because it sounds easier to tolerate can overlook the reason for the original referral.

Comparing proposed arrangements

A useful proposal identifies the purpose, exact technique, preparation, access to images and discussion after the report. Clarify who will interpret the clinical significance of any additional finding and arrange further assessment if needed. A difference in price may reflect a difference in what is included rather than the quality of the scanner.

Send the previous colonoscopy report, if there is one, together with relevant imaging. In translation, preserve the wording about bowel preparation and the segment reached. This helps the radiologist and gastroenterologist understand which part of the problem has already been investigated and what remains unanswered.

Completing the process in Turkey

An enquiry through the gastroenterology in Turkey page can include the referral and the reason standard colonoscopy is unsuitable. Coordination can help pass the documents to the receiving team. Suitability is a medical decision, and the availability of a particular protocol requires separate confirmation.

Do not base the return journey solely on the time the scan finishes. Allow for the report and a discussion before departure or an agreed remote review afterwards. If another procedure is needed, a clear handover allows the investigation to continue without repeatedly starting the medical history from the beginning.
